This information is offered with recognition that while some of the studies this information comes from is not or may not be FDA sanctioned nor approved, it is good and meaningful information. Some of the studies used comes from British, French, German, Italian, Norwegian, Japanese and Chinese sources as bio-identical hormones are not often studied here in the US. Since bio-identical hormones are not studied here in the US, we do not have many FDA approved studies with meaningful information. The reason I believe bio-identical hormones are not studied herein the US is because there is no money in it for the US pharmaceutical industry and they do not want to invest in finding this information for you. Published Negative Effects and Risk of Estradiol Treatment in Women
• Issue of Breast Cancer: Multiple studies suggest that bio-identical estrogens can reduce the risk of breast cancer when given with bio-identical progesterone. According the Women’s Health Initiative (WHI) study, synthetic horse urine estrogen does not appear to significantly increase the risk of breast cancer on its own, BUT when given with Provera or other synthetic progestins, or Provera alone, the risk fo breast cancer is increased. This risk is in the range of 1:1000 women, low. Obesity and smoking are possible associated factors but were not reported by the WHI study. Estrogens or progestins should not be used after diagnosis of breast cancer.
• Thrombosis (blood clots) Thrombophlebitis (an inflammatory process causing blood clots in one or more veins, usually in the legs): 1:400 users of HRT appear to have risk of blood clots. This is much more common with use of birth control pills as versed to horse estrogen or bio-identical estrogen. Unpredictable as who will get a clot risk is much higher in overweight women and even much higher in women who smoke. If you are not overweight or smoke cigarettes or use tobacco products and regularly exercise your risk is lower but not entirely gone.
• Ovarian Cancer: The risk of ovarian cancer is slightly higher in women using horse estrogens (Premarin). Risk with use of bio-identical estrogen haws never been established. Once this diagnosis is made estrogens should be stopped.
• Endometrial Cancer: The risk of endometrial cancer is slightly higher in women using horse estrogen. Risk with use of bio-identical estrogen haws never been established. Once this diagnosis is made estrogens should be stopped.
• Fibroid Tumors: The risk of fibroid tumors is higher in women using estrogens, estrogen should be avoided in women who already have fibroids and reduced or stopped when a fibroid is diagnosed, progesterone should be increased as it can shrink fibroid tumors.)
• Melanoma: Evidence on HRT and melanoma risk remains inconclusive. Currently believed to be a non-causal association.
• Dementia: While the KEEPS-Cog study did show that women taking horse estrogens reported improvements in mood and reductions in anxiety symptoms, there were no long term positive nor negative results in either causing nor preventing dementia.

• Women with endometriosis
• Women who are pregnant
Female Hormone Replacement Therapy Should Be Used Carefully and Monitored Closely In The Following Women:
• Women with recurring migraine or o her types of headaches which increase with estrogen use
• Undiagnosed edema or fluid retention
• Undiagnosed breast masses or recurrent breast cysts or adenomas which worse during estrogen use
• Women with very dense breasts on mammograms (may not be able to diagnose early breast cancer)
• Skin irritation if and when estrogen patch has been used in the past (may suggest allergy to estrogen)

The second key point is that while there are definite potential negative affects involved they can be monitored, watched for, prevented or treated early with appropriate follow up care.
Finally, there are far fewer, and no unrecognizable major negative side effect that can not be recognized and treated appropriately.
HRT causes few major negative risks and should be avoided in women who are at significant risk for these problems. HRT therefore should be considered by women who are at low or no risk for these problems but have meaningful negative signs and/or symptoms of hormonal insufficiency or deficiency which should be treated to return, increase or improve their overall or specific quality of life.
